As their practitioner, it's your job to help keep clients on track with their goals. One way to do this is to follow up with a goal check-in form.
This form can be used to collect information from clients on any obstacles that are preventing them from reaching their goals. You can then work with clients to address these obstacles and develop strategies to combat them. These check-ins are also an excellent opportunity to review the initial goals and adjust them to ensure they are still achievable in the set time frame. Clients can also record updates so that they can make note of any changes they may already be seeing. This form can be sent midway through the time frame you set to give clients ample time to act on any recommended adjustments to the initial goal.
Another type of form that can be created is a habit-tracking form. You can create a form to track the specific goals that clients need to accomplish on a daily or weekly basis. For example, working out a specific number of times a week or logging the number of fruits and vegetables consumed in a day.
You can use the various form-building elements to create a table that includes columns for clients to log their activity, leave notes on the task’s difficulty, and add any feedback they wish to pass along. This form can be set up with room for an entire week’s worth of data, or even sent daily. To send the form on a set schedule you can send it as a recurring form.

Recurring forms allow clients to regularly check in and reflect on progress. Having clients complete these forms at regular intervals also allows the practitioner to see if the routine and goals set for clients are realistic and achievable within the set time frame. Clients can also view their forms from week to week to see how they are progressing. While small changes won’t feel like much over a short period of time, having the client focus on what’s improving can encourage them to stick with their plan. With each completed form, the clients can celebrate the small wins each week. You can use the built-in automation within Practice Better to send the goal check-in form at regular intervals.
To ensure clients are regularly completing their forms, you can set up recurring tasks to remind them. This can help keep clients engaged and coming back to the platform by sending them notifications to complete certain tasks.
