What is an appointment reminder template?

An appointment reminder template is a pre-written text or email that tells a client when their next session is and how to confirm or change it, with blanks your scheduling system, such as Practice Better, fills in for each booking. HHS considers appointment reminders part of treatment, so you can send them without a separate authorization (HHS).

This guide gives you copy-ready templates for six moments: the booking confirmation, 48 hours before, 24 hours before, the day of the session, the intake form nudge, and the follow-up after a missed appointment. Each comes as an email and a text. They are written for wellness and therapy practices, which means they also show you what to keep out of a message that a client's lock screen can display.

Placeholders sit in curly braces, such as {First name} or {Time}. Swap them for the fields your scheduling tool uses.

What should an appointment reminder say?

An appointment reminder needs five things: who it is from, when the session is, where it happens, how to confirm or reschedule, and your cancellation window. Anything beyond that belongs in a secure channel.

ElementExampleWhy it earns its place
Who it is from"This is Riverside Wellness"Lets the client recognize the message as yours
When"Thursday, March 12 at 2:00 PM ET"Day, date, and time zone together prevent the wrong-day mistake
WhereOffice address, or "join from your client portal"Saves a reply asking for the link
Confirm or rescheduleA link, a phone number, or bothGives the client an easy way to act on the message
Cancellation window"Please give us 24 hours' notice to reschedule"Keeps your policy in front of the client before it applies

HHS gives a similar shape for voicemail reminders: to safeguard privacy, HHS suggests leaving only your name and number and other information necessary to confirm the appointment, or asking the person to call back (HHS). Apply the same discipline to text and email reminders.

If you send reminders by hand to clients in other time zones, write the zone into the message; automated reminders in Practice Better show each client their own local time (scheduling).

What should you keep out of a reminder text?

Leave out anything that tells someone other than your client why they are seeing you. If your client's phone shows message previews on the lock screen, or a family member checks the shared tablet, the reminder is the first thing they read.

HHS allows unencrypted email for treatment-related communication with patients, and it also says to limit the amount or type of information disclosed through it (HHS). Apply the same limit to texts.

If HIPAA applies to your practice, that last row is a rule. When a client asks you to contact them by alternative means or at an alternative location, HHS says you must accommodate the request if it is reasonable (HHS). HHS uses reminders as its example: a provider should accommodate a request to receive appointment reminders by email when email is a reasonable way for that provider to reach the patient (HHS). Ask each new client how they want reminders and note it in their record.

Whether HIPAA applies to your practice depends on whether you are a covered entity, which HHS explains here. The templates below are written to the same standard either way. For a full walkthrough of text messaging rules, see our guide to HIPAA-compliant texting.

Send prep instructions, results, and form names through encrypted secure messaging in your client portal, and let the reminder point there.

Appointment reminder templates by timing

Each template below has an email version (subject line and body) and a text version. The text versions stay short on purpose: they carry the time and the action, and the email carries the rest. Lines in curly braces that start with "If" are optional: keep them only when the condition fits your practice.

Booking confirmation template

Send this the moment the appointment is booked. Practice Better sends automatic confirmations when clients book, so this is the copy for that message.

Email subject: Your appointment with {Practice name} is booked

Email body:

Hi {First name},

You're booked with {Practitioner name} on {Day}, {Date} at {Time} {Time zone}.

Where: {Office address} / Join from your client portal: {Portal link}

Before your visit: you'll find any forms to complete in your portal. They take a few minutes and let us spend your session on you.

Need to change it? Reschedule or cancel at least {Cancellation window} before your appointment by calling {Phone}. {If you allow online rescheduling: You can also use the "Need to reschedule?" button below.} Our cancellation policy is here: {Policy link}.

See you soon,
{Practice name}

Text version:

{Practice name}: you're booked for {Day}, {Date} at {Time}. Details and forms are in your client portal. To reschedule, call {Phone} or use your confirmation email.

48-hour reminder template

Two days out leaves time for a client to move the appointment and for you to offer the slot to someone else. Lead with the reschedule option. In the text, {Deadline} is the last day and time your cancellation window allows a change, such as "Wednesday at 2:00 PM".

Email subject: Reminder: your appointment on {Day} at {Time}

Email body:

Hi {First name},

A reminder that you're scheduled with {Practitioner name} on {Day}, {Date} at {Time} {Time zone}.

If that time no longer works, now is the best time to move it. Call {Phone}. {If you allow online rescheduling: Or use the "Need to reschedule?" button below.} Changes inside {Cancellation window} fall under our cancellation policy: {Policy link}.

If you have forms outstanding, you'll find them in your client portal.

{Practice name}

Text version:

Hi {First name}, {Practice name} here. Reminder: {Day}, {Date} at {Time}. Need to reschedule? Please let us know by {Deadline}. {Phone}

24-hour reminder template

The day-before message is the short one. Time, place, and a way to reach you.

Email subject: See you tomorrow at {Time}

Email body:

In person: {Office address}. {Parking or entry note}

If something has come up, call {Phone} as soon as you can.

{Practice name}

Text version:

{Practice name}: see you tomorrow, {Day} at {Time}. {Office address / Join from your client portal}. Questions? {Phone}

Same-day reminder template

Keep the day-of message to one line, sent early enough for the client to plan around it. For online sessions, this is the message that gets them into the room.

Text version, in person:

{Practice name}: see you today at {Time}. {Office address}

Text version, online:

{Practice name}: your session starts today at {Time}. Join from your client portal: {Portal link}

Email subject: Today at {Time}

Email body:

Hi {First name},

Just a note that your appointment is today at {Time} {Time zone}. {Office address / Join from your client portal: {Portal link}}

{Practice name}

Intake form reminder template

Use this when forms are still incomplete ahead of a first visit. Name the action and the deadline, and leave the form names out. Practice Better's own guidance on this is to send an automated reminder if forms are not completed 48 hours before the appointment, then follow up directly at 24 hours (client engagement framework).

Email subject: A few things to complete before your visit

Email body:

Hi {First name},

Before your appointment on {Day} at {Time}, please complete the forms waiting in your client portal: {Portal link}

They take about {Minutes} minutes. Finishing them ahead of time means we can start your session with what matters to you.

Having trouble logging in? Reply to this email or call {Phone}.

Text version:

Hi {First name}, {Practice name} here. You have forms to finish in your client portal before {Day}. {Portal link}

For what goes into the forms themselves, see our guide to client intake forms.

Missed appointment follow-up template

Send this the same day a client misses a session. It offers a new time and points to your policy, and leaves out fees. For fee and waiver messages, and the policy behind them, see our guide to no-show and cancellation policies.

Email subject: We missed you today

Email body:

Hi {First name},

We missed you at your {Time} appointment today with {Practitioner name}. We hope everything is okay.

When you're ready, book a new time from your client portal: {Portal link}. Or call {Phone} and we'll find one with you.

Our cancellation policy is here: {Policy link}.

{Practice name}

Text version:

Hi {First name}, {Practice name} here. We missed you at {Time} today. Book a new time in your client portal or call {Phone}.

Therapy practice variant

If you run a therapy or counseling practice, consider a more neutral version of every text above. Drop the practitioner's name and title, and use a sender name that does not describe your services. Practice Better lets you personalize the sender name, header titles, and captions on automated emails (customizing email notifications).

Hi {First name}, this is {Neutral practice name}. Reminder of your appointment on {Day} at {Time}. To reschedule, call {Phone}.

How many appointment reminders should you send?

Send a booking confirmation plus three reminders. Practice Better sends up to 3 reminders per appointment, and here is the schedule we recommend:

WhenChannelTemplate
At bookingEmail (with a text if the client opted in)Booking confirmation
48 hours beforeEmail48-hour reminder
24 hours beforeText24-hour reminder
Morning ofTextSame-day reminder

Swap any row for the intake form reminder when a new client has forms outstanding. For the reasoning behind each touchpoint and how reminders fit with self-booking, payments, and waitlists, see how to automate appointment scheduling for wellness professionals.

The cancellation window in your templates should match your written policy word for word. If you are writing that policy now, our guide to no-show and cancellation policies has wording you can adapt. When a client misses a session anyway, no-show billing lets you charge the card on file or send an invoice in one step.

How do you set up reminder templates in Practice Better?

You set up reminder templates in Practice Better by choosing your reminder schedule, editing the default reminder email, and overriding it for any service that needs its own message. The reminder notifications guide covers each setting in detail.

  1. Set your default reminder schedule. In the Bookings and Cancellations settings, choose email, SMS, or both, and when each reminder goes out (reminder notifications).
  2. Edit your default reminder email. Paste the 48-hour or 24-hour email above into the default Upcoming Session Reminder template. It applies to all your services and clients. This level of personalization is on the Pro, Plus, and Team plans (reminder notifications).
  3. Override reminders per service. Open a service and go to its Notification Options section. A service-specific template overrides the default, and you can give that service its own reminder schedule too (reminder notifications). Use this for a first consult that needs prep instructions, or for your therapy services that need the neutral variant.
  4. Add placeholders, links, and a reply-to address. Custom email templates support basic placeholders and hyperlinks, and a reply-to address sends client replies straight to you or your front desk (customizing email notifications).
  5. Turn on self-serve rescheduling. When you enable it, clients see a "Need to reschedule?" button in their confirmation and reminder emails, and they can also reschedule from the Client Portal under My Resources, then Bookings (scheduling).
  6. Auto-send intake forms. Set forms to send automatically before scheduled sessions (automations), then use the intake form reminder above for anyone who has not finished.

Frequently asked questions

How do you write a reminder text for an appointment?

Start with your practice name so the client knows who is texting, then give the day, date, and time, and close with one way to confirm or reschedule. For example: "Hi Sam, this is Riverside Wellness. Reminder of your appointment on Thursday, March 12 at 2:00 PM. Need to change it? Use the link in your confirmation email or call us." Leave out the reason for the visit.

Can appointment reminders be sent via text?

Yes. HHS treats appointment reminders as part of treatment, so they can be sent without a separate authorization (HHS). HHS also says providers should limit the information they disclose in email and use reasonable safeguards with electronic communication (HHS). For the full rules on texting clients, see our guide to HIPAA-compliant texting.

Do appointment reminders need client authorization under HIPAA?

No separate authorization is needed. HHS states that appointment reminders are considered part of treatment and can be made without an authorization (HHS). Keeping the message to the details needed to confirm the appointment is still your job.

What should you leave out of an appointment reminder?

Leave out anything that tells a reader why the client is seeing you: the service name when it reveals a condition, diagnoses, test results, the names of the forms they are filling in, and balances owed. Send those through your secure client portal.

How many reminders can Practice Better send per appointment?

Practice Better sends up to 3 reminders per appointment by email or SMS. You can customize each reminder and set them up service by service (scheduling).

Can clients reschedule from a reminder email in Practice Better?

Yes, if you enable the option. Clients can use the "Need to reschedule?" button in their confirmation or reminder emails, or reschedule from the Client Portal under My Resources, then Bookings (scheduling).
